2012 Tornado Registration

All first time players must bring a photocopy of their birth certificate and must be accompanied by a parent or legal guardian to sign the registration form. Returning players under 18 years old must be accompanied by a parent or legal guardian to sign the registration form. Payment may be made by cash or cheque. We do not accept credit cards or debit payments.

A LATE REGISTRATION FEE of $30.00 will be applied after March 31, 2012.
A minimum Cancellation Fee of $30.00 will be applied.
A Fee of $30.00 will be applied to ALL NSF Cheques.

There will be a $125.00 refund to players that register for Rep that do not make the team.  They will then be assigned to a house league team.
